Two restaurants with double-digit violations and 8 ace reports

The following are the latest results of Portage County Health Department inspections on licensed food establishments and other types of businesses, which the Stevens Point Journal reports monthly.

Amherst

  • Go North Tavern, 152 S. Main St.: Feb. 17: One violation: South side of facility used to store building material and tools with no physical separation from the bar area.

  • O’Brien’s On Main, 128 N. Main St.: Feb. 4: Two violations: Interior of ice machine dirty; walls of walk-in cooler and exterior keg lines dirty.

Junction City

  • Brian & Wendy’s Hiawatha, 817 Main St.: Feb. 19: No violations.

Plover

  • Arby’s No. 8463, 1825 Plover Road: Feb. 4: Four violations: Employee working in food preparation area without a hair restraint; cutting boards at main prep line excessively scored; non-food contact surfaces of the bottom of the oven dirty; ice build-up on floor to wall juncture on one wall in the walk-in cooler.

  • Kentucky Fried Chicken, 1770 Plover Road: Feb. 6: No information provided.

  • McDonald’s Restaurant, 1205 Commons Circle: Feb. 17: Four violations: Chlorine sanitizer in dish machine not used according to label instructions (corrected during inspection); employee working in fryer area without a hair restraint; top of dish machine dirty and not cleaned on a frequent basis; floors of walk-in freezer and walk-in cooler dirty and not cleaned on frequent basis.

  • McDonald’s Restaurant, 2601 Plover Road: Feb. 18: One violation: Three coolers and a freezer not equipped with permanently attached thermometer.

  • Monk’s Bar & Grill, 1400 Commerce Place: Jan. 29: 12 violations: Sliced turkey, roast beef and sliced ham in six-drawer cooler not held at proper temperature (corrected during inspection); burnt ends in walk-in cooler exceeded date mark (corrected during inspection); vacuum-sealed salmon fillets not removed from reduced oxygen environment prior to or immediately upon completion of thawing (corrected during inspection); no approved or current food protection manager’s certificate posted in establishment; drinking cup without cover in food preparation area (corrected during inspection); pizza crusts in two-door freezer unwrapped or uncovered; ice cream scoop and ice cream machine components improperly stored in stagnant water (corrected during inspection); six-drawer cooler not equipped with permanently attached thermometer; six-drawer cooler not maintaining proper temperature; water temperature measuring device for wash cycle in kitchen dish machine in disrepair or not calibrated; clean drinking cups stored or stacked without being air dried; light bulbs in central light fixed in walk-in cooler not shatter resistant or shielded. Reinspection: Feb. 10: Four violations: Pizza crusts in two-door freezer unwrapped or uncovered; water temperature measuring device for wash cycle in the dish machine in disrepair; clean drinking cups stored or stacked without being air dried; light bulbs in central light fixture in walk-in cooler not shatter resistant.

  • Stevens Point Area Pizza Ranch, 1300 Commerce Place: Feb. 24: Eight violations: No “time as a public health control plan” is available for review; temperature measuring device for wash cycle of dish machine is not functioning; no irreversible registering thermometer provided for measuring surface temperatures in the hot water ware washing operation; soap dispenser at handwashing sink near ware washing area is missing front cover and lever to dispense soap; flooring in walk-in cooler and freezer dirty; crumb tray on pizza oven soiled; right sink in women’s restroom not functioning; mops stored in basin of mop bucket.

  • St. Adalberts Parish, 3315 St. Adalbert Road: Feb. 27: One violation: No irreversible registering thermometer provided to measure the surface temperatures in the hot water mechanical ware washing machine.

Stevens Point

  • Burger King No. 25898, 5431 U.S. 10: Feb. 17: Three violations: Test strips not available to monitor treatment for raw vegetables; door to walk-in cooler not properly adjusted; light intensity in walk-in cooler not bright enough.

  • CPS Cafeteria, UWSP College of Professional Studies Building: Feb. 21: One violation: Cinnamon rolls and carrot cake held at the customer serving line not protected from contamination.

  • Delta Dental of Wisconsin, 3100 Business Park Drive: Feb. 7: No violations.

  • El Molcajete Mexican Grill, 1160 Meridian Drive: Feb. 3: 11 violations: Pork, chicken breast, Mexican cheese and flan not held at proper temperatures (corrected during inspection); salmon fillets not removed from reduced oxygen environment prior to thawing (collected during inspection); test kit not available for testing sanitizer concentrations; ice machine drip plate and bar soda gun nozzle dirty (corrected during inspection); handwashing sink by dish machine only had cold water (corrected during inspection); multiple food items in walk-in freezer unwrapped and uncovered; wiping cloths used for wiping surfaces stored on sides of sanitizer buckets in kitchen and waitress area (corrected during inspection); four-drawer cooler not holding proper temperature; clean metal pans stored stacked without being air dried (corrected during inspection); handwashing sink by food prep area not operational and bar left ware-washing sink handle leaking; light bulb above grill not working. Reinspection: Feb. 25: One violation: Handwashing sink in food prep area not working and bar left ware-washing sink handle leaking: Note: Owner ordered and received the parts to repair sinks and planned to fix them the week after reinspection.

  • Library Cafe, 501 N. Michigan Ave.: Feb. 6: One violation: Whole fresh fruit in service area not protected from potential contamination.

  • McDonald’s Restaurant, 5625 U.S. 10 E.: Feb. 25: No violations.

  • Pizza Hut, 417 Division St.: Feb. 26: Four violations: Pizza toppings held in cooler not date marked; test kit not available for checking sanitizer concentration; drain tray of soda machine dirty (corrected during inspection); wet mops stored in basin of mop sink or on floor.

  • Point of Discovery School, 1900 W. Zinda: Feb. 28: No violations.

  • Rookies Kitchen, 3425 Church St.: Feb. 10: Seven violations: Single-use plastic portion cups being reused to dispense shredded cheese, and cups stored directly in package of shredded cheese (corrected during inspection); whipped butter packets stored at room temperature (corrected during inspection); several ready-to-eat foods stored in refrigerators have exceeded their date marks (corrected during inspection); wiping cloth sanitizer solution not being used according to label instructions (corrected during inspection); consumer advisory missing disclosure statement; interior cavity of microwaves in kitchen and at wait station dirty; clean metal food pans stacked and stored without being air dried.

  • Rookies Sports Pub, 3425 Church St.: Feb. 10: No violations.

  • St. Joseph Parish, 1709 Wyatt Ave.: Feb. 28: No violations.

  • The Waters Pub & Grill, 736 Old Wausau Road: Feb. 15: No violations.

Wisconsin Rapids

  • Chat-R-Box, 3821 S. 80th St.: Feb. 18: No violations.
